When and why do we perform endodontic treatment on teeth?

There are several indications for endodontic treatment. However, the list includes primarily:

  • deep carious lesions, pulp necrosis, pulp gangrene,
  • mechanical tooth injuries (dislocations, fractures),
  • planned prosthetic treatment,
  • inflammation of periapical tissues,
  • incorrectly performed first endodontic treatment (periapical lesions, leaking canal fillings, fragments left in them tools, etc.).

You also need to consider the economic aspects of root canal treatment of a tooth, versus possible extraction and subsequent implant placement or filling the gap with a prosthetic structure such as a bridge.

What is endodontic treatment (root canal treatment)?

The individual stages of endodontic treatment:

  1. X-ray of the tooth before treatment gives the doctor information about the anatomy of the canal system. Thanks to this, it is known how many canals there are, whether they are patent, obliterated, curved or straight.
  2. Local anesthesia of the patient guaranteeing the painlessness of the procedure.
  3. Placement of a rubber dam , i.e. an insulating material used in dentistry instead of lignin rolls. It consists of a thin, usually latex rubber, in which there is a hole through which a selected tooth or teeth are isolated. A clamp is placed on the rubber, there are several types, selected depending on the size of the tooth. At the end, it is stretched over a frame placed around the patient's mouth. The rubber dam completely isolates the tooth's canal system from the bacteria in the saliva.
  4. Opening the tooth chamber and removing the pulp using the appropriate size of rose-head burs.
  5. Finding the canal orifices - this is a very important stage, because omitting a canal results in leaving bacteria, which can lead to post-treatment complications in the form of pain and, consequently, to the need for re-treatment of the tooth root canal or its removal. A microscope is very helpful here.
  6. Mechanical widening of the root canal orifices.
  7. Gaining access to the canal in a straight line by removing the dentin overhang above the orifice with a bur reduces the risk of breaking the file during chemo-mechanical preparation of the canal.
  8. Determining the working length - another responsible stage; the tooth must be prepared to the physiological foramen, i.e. the constriction inside the root canal, which the pulp proper reaches. The physiological foramen is usually located 0.5 - 2 mm from the root apex and is the border to which the canal should be prepared and filled.
  9. Chemo-mechanical preparation of the canals, which is carried out using special nickel-titanium canal instruments. They are specially designed to safely open even curved canals and give them a conical shape. Giving the canals such a shape is a condition for their proper filling. This process is supported by abundant rinsing of the canals with chemical agents (the basic agent is 5.25% sodium hypochlorite), which are intended to disinfect and help clean the entire canal system.
  10. Filling the canals. After preparing, cleaning and widening the canal and drying it thoroughly, the doctor proceeds to fill them. This stage is aimed at tightly closing the canal lumen along its entire length with a material specially designed for this purpose.
  11. Control X-ray shows whether the canals have been correctly filled to the apex.

Additional instruments needed to perform endodontic treatment are:

  • Micromotor for mechanical canal preparation - these are systems of special burs for preparing root canals, even very curved ones, and giving them the appropriate width and shape so that the filling is as accurate as possible.
  • Microscope - Working at a very high magnification allows you to avoid missing additional root canals in a given tooth or various types of side orifices located in the main canal.
  • Endometer - Precise measurement of the working length of the root canal.
  • X-ray - Radiological control to confirm whether the canal has been filled correctly.

The effectiveness of the procedures is increased by the use of a treatment microscope. Thanks to which an experienced doctor very precisely cleans the canals of dead tissue.

An X-ray taken after root canal treatment allows for an immediate assessment of the effectiveness of the treatment. Ultrasonic tools with a diamond coating allow us to reach impassable canals.
